Short version: Mort Rainey (Johnny Depp) is the killer. John Shooter (John Turturro) doesn't exist except in Mort's mind.
Long version: Mort Rainey is a successful but frustrated writer in a remote cabin in upstate New York. John Shooter claims that Mort plagiarized his story "Secret Window," and wants him to fix the ending. Mort doesn't realize that Shooter exists only in his mind: a character/personality who does Mort's dirty work; kills his dog, his lawyer (Charles S. Dutton), and his almost-divorced wife Amy (Maria Bello) and lover Ted (Tim Hutton). Mort realizes he is Shooter before he murders Amy and Ted, and changes the ending of "Secret Window" to follow what happened in real life: he buries Amy and Ted under the corn in the garden. Their bodies are not found, Mort has not been convicted, but the town sheriff warns him that it's just a matter of time.
