Charlie (Jim Carrey) overcomes his mental issues and vanquishes the “Hank” personality, also saving Irene (Renee Zellweger) and losing a thumb in the process. Dickie (Daniel Greene) is killed, and dirty cops Gerke (Chris Cooper) and Boshane (Richard Jenkins) are arrested. In the end, Charlie pulls off a ruse to propose to Irene, she accepts and everyone lives happily, ever after -sort of-.

Longer version
The constant emergence of “Hank”, caused by Charlie having accidentally forgotten his medication to control “him”, reaches a peak when “Hank”, pretending to be Charlie, convinces Irene to have sex, which Charlie is annoyed to discover later, while they are both on the run. Gerke and Boshane -both on Dickie’s payroll and conducting the manhunt to catch Charlie and Irene, and eliminate them- almost get a hold of them at the motel, but they elude the capture, while, simultaneously, Charlie’s three black adoptive sons -Jamal (Anthony Anderson), Lee Harvey (Mongo Brownlee) and Shonte Jr. (Jerod Mixon)-, steal a police helicopter and mislead the two crooked cops through a false report about “their targets having been caught nearby”.

Charlie and Irene decide to leave “Whitey” (Michael Bowman) -whom Charlie believed to be a deranged murderer who killed his parents in their sleep- at the motel and board a train back to Rhode Island, unaware that Dickie, by recommendation of Gerke and Boshane, has boarded the same train to kill them himself. He does manage to catch and kidnap Irene; Charlie, with “Hank” manifesting at the same time, chases after him and both personalities reluctantly join to rescue Irene. The chase leads them all to a bridge where Dickie threatens to kill Irene and throw her off; “Hank” hesitates and tries to deter Charlie from taking action, but Charlie finally stands up to “Hank” and faces his issues and fears, defeating “him” and confronting Dickie.

In the struggle to disarm Dickie, a fire is shot, blowing off one of Charlie’s thumbs, which fazes him. When it looks like Dickie will finish Charlie off, Whitey, having followed the whole group, kills Dickie from afar by impaling him with a land dart. Charlie and Irene fall into the river, but Charlie’s sons rescue them both, also reuniting with Whitey, who admits he made up the murder backstory because he felt freaked out by “Hank”. When the police arrive, now knowing the truth about Irene, they arrest Gerke and Boshane, Irene’s name is cleared and Charlie is praised for his newfound courage and efforts. However, Irene appears to be ready to leave Rhode Island, but the State Troopers once again pull her over, only to reveal that Charlie planned it to propose to her; she gleefully accepts.

The movie ends with Charlie’s sons and Whitey flying a plane together which shows a comical banner congratulating Charlie and Irene, as they break the fourth wall and say “Bye!! Thanks for watching our motherf***king movie!!”. The credits roll, showing pictures of the actors -including those whose scenes got cut-, and, in a post-credits scene, the whole gang gets together to try and rescue Charlie’s thumb from the river; Whitey spots it but, as he is about to tell the others, a fish appears out of nowhere and swallows it whole, to which he just retorts “Never mind”.
