Homer (Tobey Maguire) finally comes to senses and realizes that his destiny was indeed to become a doctor and follow Dr. Larch’s (Michael Caine’s) steps.  This takes place when, one night, Rose Rose (Erykah Badu) tries to escape, to hide her incestous pregnancy -and, of course, the tremendous shame she felt- from everyone, but her father -the culprit-, Mr. Rose (Delroy Lindo) won’t let her leave.  Homer approaches them and offers to “help” -that is, to perform an abortion-.

The procedure takes place successfully.  However, a little time later, Mr. Rose, overcome by remorse and guilt, stabs himself, and dies shortly after the orchard crew and Homer find him.  At the same time, Homer receives the unpleasant news that Dr. Larch also died, from an accidental overdose of ether -to which he was addicted to-, and that the orphanage has been left with no headmaster; furthermore, Capt. Wally (Paul Rudd) is brought back from the War, paralyzed.

Homer leaves with the orchard crew, and he and Candy (Charlize Theron) exchange sad looks while he is driven away.  Eventually, he makes his triumphant return to the St. Cloud’s orphanage, where he is received as a hero, and takes his place as the new doctor, caretaker of the children, and administrator.
