Theresa (Julianne Moore) is terminally ill and knew of the connection between Isabel (Michelle Williams) and Oscar (Billy Crudup); that Isabel is the biological mother of their daughter Grace (Abby Quinn).

After Theresa makes a larger endowment to Isabel’s organization contingent on her staying in New York, Theresa reveals her diagnosis to her and her hopes that Isabel will stay and be a mother to Grace and a mother figure to her twin boys. Though initially angered and hurt by the manipulation Isabel eventually agrees to the terms, unwilling to turn down the help and having slowly gotten close to her daughter.

Oscar and Grace eventually learn of Theresa’s illness and are devastated but understanding of why she’d rather have quality of life over a prolonged illness. However, shortly after a goodbye dinner with her employees, the stoic Theresa, who had worked so hard to get her affairs in order and make sure her family was okay, breaks down sobbing in her husband’s arms, saying she doesn’t want to die and that she is “not done.”

A short time later, Theresa dies and the family, along with Isabel, spread her ashes on the family property. Isabel returns to the orphanage, and tells Jai, a boy she was close to there, that while she must now stay in New York, she will do her best to visit when she can.
