In the end, Cassie (Melissa Sagemiller) wakes up from a two-day coma, and discovers that Sean’s (Casey Affleck’s) death was all a dream. The ones who survived in that dream, Annabelle and Matt (Eliza Dushku and Wes Bentley), both actually died.
